Self Assessment

Substance use disorder (SUD) is a disease that affects a person’s brain and behavior, leading to an inability to control substance use. Those with SUD may use legal or illegal substances such as alcohol, marijuana, prescriptions and opiates among others. This substance use disorder self assessment will help you determine if you should seek help for SUD.

Answer the following yes/no questions as honestly as possible to find out if you might have SUD.

  • Do you often use substances more than you plan to?

  • Have you been unsuccessful at quitting the use of substances in the past?

  • Has your substance use interfered with your personal relationships?

  • Has your substance use interfered with your job?

  • Do you feel regret or remorse after using substances?

  • Do you avoid social situations where you cannot use substances?

  • Do you frequently have hangovers or other after-effects of substance use?

  • Do you prefer using substances over other activities you used to enjoy?

  • Are you experiencing health issues related to your substance use?

  • Do you find yourself needing to consume more substances now in order to feel the same effect as before?

  • Do you find yourself craving substances?

  • When you stop using substances, do you experience physical or emotional discomfort?

  • Do you often lose memory while using substances?
